If Purchaser needs to return a product, please contact Fortna within 10 days of receipt of the Purchase. Fortna will provide an Return Material Authorization (“RMA”) and give Purchaser the address for the return. Purchaser is responsible for shipping costs for the return. Fortna may issue a credit excluding freight charges after an inspection of the Purchase has been completed. Purchaser must ship the returned Purchase in appropriate packaging to avoid damage, freight prepaid, unused, and in resalable condition. The returned Purchase is also subject to a 25% restocking fee that will be applied against the return credit to the Purchaser. Fortna reserves the right to refuse return credits on damaged, used, or non-resalable items.

- Purchaser Name
- Invoice Number
- Part Number
- Serial Number (if applicable)
- Reason for return
